HHCSRC Inc. [Holistic Health Care Society & Research Center Inc.] CONTRACT OF DISCLOSURE, INFORMED CONSENT, DISCLAIMER, TERMS, CONDITIONS & REGISTRATION 1. I understand that, HHCSRC Inc. ONLY sells “Dietary Supplements” NOT PRESCRIPTION MEDICATIONS. The Dietary supplements have NOT been evaluated by FDA. Dietary supplements (D.S.), skin & hair care product of the HHCSRC Inc. contains herbs, vitamins, minerals, amino acids, enzymes etc. 2. I understand that, The HHCSRC Inc. and Dr K. Rao cannot answer personal and specific health questions. They do not give advice, make recommendations, suggestions, claims, or guarantee a possible period and which supplements one should take. These types of questions are best reserved for my health care provider. Our conversation and opinion is only about dietary supplements of HHCSRC Inc, and for informational purposes only. 3. I hereby agree that I have been well informed and know all details as well as pros and cons about these Dietary supplements I will purchase from HHCSRC Inc. I know that these Dietary supplements are not intended to diagnose, treat, cure, or prevent any disease or medical condition or to replace professional medical advice/treatment. I understand that there is no assurance or guarantee of any kind regarding any benefit from these dietary supplements. I have been told that this product may contain other ingredients unintentionally introduced in the raw material that was used to create the supplement, or during harvesting, or during the manufacturing process. It may cause an allergic reaction. 4. I understand that no one can guarantee that these supplements will benefit me. I know that each person is unique in his or her response. One person may notice great benefit from a supplement, while another may have a side effect or an allergy. I understand that there is no guarantee of any kind regarding any benefit and period of time and no claims is made about benefits of these dietary supplements. Also, individual results may vary & I know that they may or may not be of help to me. I am taking them on my own responsibility. I understand that, HHCSRC Inc. and Dr K. Rao only supply these dietary supplements specifically ordered by me. 5. I agree that I am using these dietary supplements as per best of my knowledge & nobody will be held responsible for any kind of side effects which might have occurred because of my health problems or the long term using of traditional medications or these dietary supplements. 6. I understand that, Individual results vary with dietary supplements & they may or may not be suitable to me. I have been repeatedly told by HHCSRC inc. that I should discuss about my using these Dietary supplements with my family members and health care physician for its effects and any drug interaction and take their consent before I purchase them. I am also told that I should not to stop my prescription medicines without the consent of my health care provider when I start using these dietary supplements. 7. My health problems are very complex, chronic & unable to be controlled by medications and due to side effects of prescription drugs. Due to this, I may develop acute complications that may coincide with the use of these supplements. 8. I understand that, Dr K. Rao is just an employee of HHCSRC. Inc and does not own this corporation. I know that my contract will be with HHCSRC Inc. I am doing payment to HHCSRC Inc. for the purchase of dietary supplements. Therefore, any future disputes will be restricted to HHCSRC Inc. NOT personally with Dr K. Rao. 9. I understand that this website is not any medical doctors’ or natural doctors’ website to seek any medical services likes physical examination /investigation/ medical advice/ prescription/ physiotherapy, etc. I understand that Dr. K. Rao serves as the Science Director of the HHCSRC Inc., which sells dietary supplements and not as its medical practitioner. [Dr. Rao has a Bachelor of Medicine and Bachelor of Surgery Degree from India, which is the equivalent of an MD in the USA. However, he is not a licensed medical doctor in the USA and does not advice, examine, diagnose, cure, and treat any disease or recommend dietary supplements. Dr Rao only provides Dietary Supplements products for purchase by the customers who visit the center. He is a member of the American Academy of Pediatrics but as he does not wish to practice prescription medicine hence has not applied for and completed the board certification and licensing in the USA.] I understand that, all material provided on the site is provided for informational or educational purposes only. I understand that all communication and discussions about the dietary supplements are strictly for information purposes only. I understand that HHCSRC Inc. does not provide emergency services. I understand that if I need a medical diagnosis or treatment, I should contact the County Medical Society for referrals. I understand that at HHCSRC Inc. or Dr. Rao do not examine, diagnose, advise lab tests or treat customers medically and do not sell or prescribe medicine. 10. I do not have any objections and authorize for the use of my investigation reports, disease progress, and audio-visual recording being used for the betterment of people, scientific studies, publication, advertisement, Television shows, internet, websites and other Media outlets & research purposes. I have agreed on my own will to record clipping regarding my view of these dietary supplements for this purpose. I also have agreed for my friend/ family member/ guardian to express his/her view about these supplements in these clippings. I or my family and friends or legal representatives will not take any kind of legal action against HHCSRC Inc., media agency for publishing and broadcasting this clipping. I agree that HHCSRC Inc. has the sole copyright of these recordings and reports and I have given them the authority to use the recordings as and when required. Relatives, my legal representatives, friends or I will not take any kind of legal action against the HHCSRC Inc. and/or Dr. Rao, and any media agency for publishing and broadcasting these clippings. 11. Return/Refund Policy- I understand that, at HHCSRC Inc., all sales of merchandise are final and there are No Guarantees, No cancellation of orders, No returns, No refunds and No exchanges. Therefore, once payment is done I cannot dispute this payment transaction anywhere, including but not limited to the Credit Company, bank or court of law. I understand that, HHCSRC Inc. does not accept any personal check. I understand that I will be responsible for any check decline, bank charges, stop payments, check returned and be liable to repay the charges with dues, including attorney’s fees, legal charges and/or collection agency fees. 12. I understand that, I have to pay an initial fee of $100.00 for discussion, which is non-refundable. I understand that, I have to pay separately for whatever supplements I want to buy along with shipping & handling charges. 13. I understand that, any legal disputes/litigation will be only between me (buyer) & the HHCSRC Inc. This excludes Dr K. Rao, any employees and volunteers personally, as all these are only employees of the corporation, and all disputes shall be resolved only within the Jurisdiction of the HHCSRC Inc.’s present location county court at the time of filing of litigation. I hereby agree, not to elect trial by jury of any issue fully, knowingly and voluntarily and is intended to encompass individually each instance and each issue as to which the right to a trial by jury would otherwise accrue. HHCSRC Inc, and its representatives are hereby authorized to file a copy of this paragraph in any proceeding as conclusive evidence of this waiver. 14. I have understood in detail about the pro and adverse effects of dietary supplements. I have thought thoroughly about buying these dietary supplements and only then have made my decision to purchase these dietary supplements. I understand that these dietary supplements are mixed for my over all benefit and once prepared for me cannot be reused for someone else. Therefore, it cannot be cancelled/exchanged/ returned/ refunded. I understand that I will be totally responsible for my purchase and myself, my relatives, friends or legal representatives can’t ask for return/replacement/ refund in future either on phone/e-mail or in person. 15. I understand that, the supplements from the HHCSRC Inc. will be consisting of powdered supplements, extracts or concentrates and may be found in many forms, such as tablets, capsules, soft gels, gel caps, liquids and powders. 16. I understand that, in future, if there is any need for discussion regarding the dietary supplements (No discussion about replacement/return/refund) only the user of the dietary supplements can call, or fix a time for visit, so that I can be given a convenient time for discussion. I also understand that it is not possible to explain details to well wishers/friends. I know that whenever I call I should give a time for return call, as they have to be answered by authorized personnel only. I have been told that during weekends and holidays or when there is problem with telecommunication there may be delay in getting an answer. 17. I understand that, sales and services rights are reserved by the HHCSRC Inc., and its services can be denied if necessary. 18. I understand that, supplements are prepared for one month/ months supply. After full payment, the supplements will be prepared and supplied the same day. I understand that, prepared supplements have to be picked / shipped within 7 days; after 7 days, the order will be discarded and the HHCSRC Inc. and/or its employee cannot be held responsible. I understand that, supplements are best used within 3 months of preparation. 19. Important Note For advisers/well wishers: People come to us after exhausting all available options. It will take time for these supplements to take effect. We highly appreciate each dollar spent from your hard earn money and sincerely hope you will get the desired effects from the supplements you purchase. We cannot guarantee a supplement will help you; one may notice great benefit from a supplement, while other may not have desired benefit or may have a problem. However, we can tell you that the products we are offering are quality products. There is no way we can please everyone with every product. So please think of the pros and cons and if you feel that these supplements can help you then please call us and pay us a visit at a mutually convenient time for both of us. Otherwise, we firmly request you to continue only with your current routine that you are already taking in abundance. Rarely some people do change their mind or under the guidance of somebody else who do not know anything or do not have faith about the dietary supplements try to abuse the system for replacement/ return/ refund. When you actually think about pharmaceutical drugs prescribed by your doctor, you will realize that neither the pharmacy nor the drug company will accept a return of a bottle if the antibiotic or other medicine does not work or if you simply decide you do not want it after some time. These are food supplements, not prescription medications; and once prepared cannot be used for anybody else. Therefore, we again request you to understand everything before signing this form and before placing an order. Reading glasses with different power are available to read these contract papers, if needed. I have read the CONTRACT OF INFORMED CONSENT, DISCLAIMER, TERMS, CONDITIONS & REGISTRATION thoroughly WITHOUT ANY RUSH, understand the facts before signing this contract. I am buying the dietary supplements (They have NOT been evaluated by FDA) NOT PRESCRIPTION MEDICATIONS) of my own free will after fully understanding the above facts. HHCSRC Inc. or its employees and volunteers have not done any fraud, misrepresentation, breach of trust or undue influence of any kind on me to extract money. I agree that I will not intentionally cause any damage to HHCSRC Inc. physically, financially or in reputation. I hereby agree that my heirs, executors, administrators, personal representatives, successors or assigns and I are legally bound, by this contract. I understand that I am buying the dietary supplements NOT PRESCRIPTION MEDICATIONS and if there is any problem or any kind of disagreement or if I do not trust dietary supplements can benefit then I should not buy any products from HHCSRC Inc. I am buying the dietary supplements after fully understanding the above facts. I am not under the influence of Alcohol or any other drug or under Pressure of any other person. I am signing on this form willingly so that it can be used wherever required legally.

As soon as the Order is placed the mixing of supplements will be done and once they are mixed for buyer they cannot be either separated or used to any body else.
